The scheduled job information sheet provides us with the technical information we need in order to prepare for your job, as well as, the legal reporting requirements. This form must be completed at least 24 hours before we start your job. However, it can be completed well in advance of that time frame.

Simply download these forms, fill in the necessary information and mail or fax them to our office (503)692-4759.

Note: You must include the Required Site Information Form and Utility Notification forms at the bottom of the lists including your Locates Ticket number, contact and contact phone number.
